This text, a compilation of views from an international panel of experts, represents the state-of-the-art in hernia surgery An overview of current thoughts and practice in abdominal wall hernia repair is given.

The text begins by discussing fundamental concepts; such as indications for surgery, assessing the outcome of surgery and economic implications. Local anaesthesia groin hernia repair and prosthetic materials currently used are also reviewed. With particular reference to sportsmen, groin pain in the absence of a palpable lump is addressed. Recurrent groin hernia repair is examined, and difficult cases, initially considered inoperable, are highlighted.
